As your insurer, Pan-American Life Insurance Company (Pan-American Life) may collect certain health information from you, either from your insurance application for coverage or from yourself or your provider for reimbursement of medical expenses.

The law permits Pan-American Life to share this information under certain circumstances without your written consent: for example, with a Third Party Administrator or other claims management individuals who need the information to administer your claim.

Pan-American Life must also, from time to time, disclose your personal information without your consent for legal reasons, such as to respond to a subpoena issued by a court of law or to a government agency conducting a criminal investigation.

Pan-American Life will not release any of your personal medical information to anyone other than as permitted by law without your prior written consent. In addition, under certain conditions your health information may be disclosed to a plan sponsor for statistical, renewal or quotation purposes. However, your plan sponsor is not permitted to use such information for employment purposes.

You have the right to request restrictions on certain uses and disclosures of your protected health information; however, Pan-American Life is not required to comply with such requests. We will make every effort to do so, however.

You have the right to inspect your file in our office and to make copies of your file.

In addition, if you send Pan-American Life a request in writing, we will give you details regarding any disclosures of your medical information while it has been in our possession.

If you elect to receive any or all of the above information electronically, you will always have the right to request paper copies of the information as well.
